A 23 year old patient presented to the emergency room because of a penetrating nail gun injury to his left chest. A nail gun had been accidentally fired one meter away from him, hitting him in the chest. His only complaint was chest pain and he was in stable condition. A CXR and CT scan of the chest [Figure 1] & [Figure 2] showed the nail going through the 8th intercostal cartilage through to the heart. At surgery, the nail was extracted. It was lodged in the costal cartilage, penetrating through to the heart. There was laceration of the cardiac apex which was repaired. The patient did well and was discharged after four days.
