Although ischemic cardiomyopathy and diastolic dysfunction account for most of the cases of clinical heart failure, constrictive pericarditis should be entertained in the differential diagnosis [Figure 1], particularly when right-sided failure is predominant. Surgical pericardiectomy, if feasible, is the treatment of choice.{Figure 1}
